The San Francisco Foundation Funds SteppingStone's Innovative Community Based Health Home Pilot
SteppingStone is pleased to announce that we have received $150,000 from The San Francisco Foundation for its Community Based Health Home (CBHH) pilot project. The CBHH is an emerging model for stabilizing health and psycho-social risk factor for older adults in Adult Day Health Care (ADHC) whose complex conditions are exceptionally challenging and put them at risk for higher levels of care. The primary goal is to create a strong effective Health Home by using ADHC as the core hub for coordinating care for the MediCal population. The model extends the specialized services available at SteppingStone's ADHC centers into the client's home, community and constellation of care by using a RN Navigator. The client is at the center of the CBHH offering care coordination for those low-income seniors with highly complex medical, cognitive and behavioral health conditions who are at high risk of institutional placement.
SteppingStone believes that the CBHH model, once fully developed and tested, will help to inform California's development of Patient-Centered Medical Homes through the 2703 state plan option under the Affordable Care Act, as well as improve outcomes for vulnerable high-risk individuals in need of support for mental health issues. The CBHH will significantly reduce overuse of emergency rooms and avoid unnecessary hospitalizations and readmissions, key problems that the healthcare system is working to address.

SteppingStone Adult Day Health Care as the largest and most experienced Adult Day Health Care provider in San Francisco is celebrating its 30th anniversary enabling thousands of frail seniors and disabled adults to live independently at home while thriving in a community that promotes health, happiness, and quality of life.
Founded in 1983 as North & South of Market Adult Day Health by the Curry Senior Center, SteppingStone was the first Adult Day Health Care provider in the country to develop the innovative model of co-location with affordable senior housing through its partnership with Mercy Housing and TODCO. It also provides critcally needed respite for family member caregivers, allowing them to go to work knowing that their loved one is happy and being well cared for. The organization is a recognized leader in San Francisco and statewide as an advocate for protecting and defending the rights of seniors and disabled adults.
The California Association for Adult Day Services (CAADS), a statewide advocacy group that advocates for the development of adult day services chronically ill and disabled adults awarded their annual Community Advocacy Award to Benny Wong, Program Director of SteppingStone’s Golden Gate Day Health Center.
Benny was chosen among leaders of the over 300 Adult Day Health Care (ADHC) centers in California because of the extraordinary empowerment he has nurtured at the Golden Gate ADHC in San Francisco’s Tenderloin neighborhood. Benny energized the ADHC participants to speak out to their district legislative representatives and to the Governor protesting against proposed cuts and elimination of ADHC in this past year’s budget proposal.
